Opportunity for Patient Care Coordinator on Fixed Term Contract for our hospital in Mahim.

Duties & responsibilities:
- Assistance to Patient enhancing patient satisfaction and aid conversion to Out-Patient Services / In-Patient Services; investigations and treatment.
Assigned to:
- Specialties like Oncology, Neuro Sciences, Orthopedics in the initial phase, could be emulated depending on succes
- To coordinate, ensure smooth end-to-end facilitation of visiting patients process, be an active enabler; aligning various service providers towards the goal of overall Patient experience.
- Guiding, escorting visiting Patients and provide a seamless Care process
- Guiding on pre-requisites, to ensure unnecessary movement over floors configured
- Referral Management, with respect to investigation as an outcome of the consult
- Coordinate with the relevant service providers to ensure quality service delivery
- Providing scheduling/appointment and other process related information, in a language the
Patient is comfortable in
- Answering Patients queries and providing clarification as an when required throughout the course of their process
- Coordinating between Patients and Hospital stakeholders to enable / facilitate
- Knowledge of care module for booking appointments, billing / tariff, reports & pre requisites, as required
- To work amicably, closely liaison with various service provider teams including Doctors,
Technicians, Nursing staff, Clerical, Attendants and other health professionals in a
collaborative manner
- To be Section Facilitators for the assigned wing/ floor/ Specialty
- To abide by Hospital Rules and Regulations so as to ensure department working is not hampered, visiting Patients not inconvenienced
- To do miscellaneous activities as and when directed by Reporting Manager
